Funded by the Vermont Blueprint for Health, the North Star Community Health Team has programs and services to support you in preventing and managing chronic conditions (like diabetes and high blood pressure), reaching your personal health goals, and overcoming physical, financial, social and emotional barriers to living your healthiest life.

Community Health Team services are free and available to anyone who may benefit from them. Talk with your primary care provider about a referral for Community Health Team services, or call the North Star Community Health Team at the health center where you receive care.

      About Me
      Growing up I had the opportunity to witness family members providing excellent care to people in different ways. I’ve always enjoyed delving into a complex situation to determine the right course of action. North Star Health employees have been providing excellent care to our community and I’m honored to be a part of this team. I grew up in Claremont and have lived in Vermont for 11 years now. This area is quite familiar to me. I enjoy spending time with family, my 7lb dog and 29lb cat and friends. I enjoy a wide variety of music and middle eastern dance.
