North Star Health offers financial assistance in the form of a sliding fee discount program. Eligibility is based on family size and annual income. Payments for eligible individuals are adjusted according to a sliding fee schedule that is based on current federal poverty guidelines.
Unless indicated otherwise on a provider’s North Star Health website profile, all North Star Health clinical providers are included in the North Star Health sliding fee discount program.

- Contact Valley Health Connections at 802-885-1616 or [email protected]. They offer free, confidential assistance and can help you apply for North Star Health’s Sliding Fee Discount. They can also help you enroll in a health insurance plan, Medicaid, and/or other financial assistance programs you may be eligible for.
- Download and print the North Star Health Financial Assistance Application. Take it with you to your Valley Health Connections appointment, or complete it and return it to us.
