Free diabetes education and support services are available through the Community Health Team to people who have or are at risk for diabetes. Whether you are newly diagnosed or have been living with diabetes (or prediabetes) for a while, our skilled professionals can provide you with tools, health tips and food ideas to help you live a healthier life and reduce your risk for complications.

Following the guidelines of the American Diabetes Association and the Association of Diabetes Care and Education Specialists, our diabetes education team provides education and support in the following areas:

  • Healthy eating
  • Being active
  • Monitoring blood sugar
  • Taking medication
  • Problem solving
  • Reducing risks
  • Healthy coping strategies
