Patients enrolled in Medicare Part B are eligble for a no-cost Annual Wellness Visit.
Purpose
The purpose of the Annual Wellness Visit is to create or update your personalized health plan. It is intended to help you stay health, safe, and up-to-date. It is not a routine physical exam.
What to Expect
- Review of your medical history and medications
- Questions about your overall health and daily activities
- Screening for fall risk, memory changes, and depression
- Review of recommended preventive screenings and vaccines
- Optional discussion of advance care planning
What NOT to Expect
- Physical exam
- Evaluation or treatment of new symptoms
- Management of chronic medical problems
- Medication changes for medical concerns
- Routine lab work or imaging unless ordered separately
Cost
There is no cost to you if the visit is preventive only. If medical problems or symptoms are discussed and treated during this visit, Medicare may bill that portion separately, which could result in a co-pay or charge. How to Avoid Unexpected Costs

If you have new symptoms, chronic condition concerns, medication questions, or other medical issues, please let us know when scheduling or checking in. We can schedule an additional office visit, so you do not have to burden the cost of an additional medical bill, and your provider has enough time to fully address those concerns.
How to Prepare for Your Visit
- Bring a current list of your medications or medication bottles
- Bring your glasses and/or hearing aids
- Bring copies of advance directives if you have them
The Medicare Annual Wellness Visit is available once every 12 months for patients with Medicare Part B.
